Menstrual migraines.

Continued. Before you read this article, please read the first part, Hormones and migraines.

Menstrual migraines also can be stopped by prescription drugs. Here is the list of these medications:

  • Depakote - anticonvulsants, like valproate
  • Cafergot, Migranal, Bellergal-S - drugs, containing ergotamine
  • propranolol - beta-blocker drugs
  • verapamil - calcium channel blockers

  • Like NSAIDs medications these prescription drugs should be started three days before the beginning of menstrual cycle and continued until the period ends.

When different types of treatment have been used without any results, drugs (like Lupron), that change hormone level are used.

Sometimes menstrual migraines can be caused by fluid retention, which is associated with a menstrual cycle. In that cases diuretics are used for menstrual migraines treatment. Decreasing salt-intake in the beginning of woman's period also can help.

For those who have to use post-menopausal estrogen supplements should keep taking them on a regular basis, but only small doses. They have to be taken every day. Keeping constant level of estrogen doses migraines can be prevented. The level of estrogen can be regulated by Estraderm - estrogen patch.

    Pregnancy migraines treatment.

No drug therapy for migraines is recommended during pregnancy. Migraine treatment medications can have negative effect on a germ, that's why they are forbidden during pregnancy. Hormones and migraines.

According to researches more than 65% of those who have migraines are female. In most cases the reason of migraines is menstrual cycle. That's why they are called menstrual migraines.

What Headaches and Hormones have in common?

In the beginning of a woman's menstrual cycle the level of female hormone estrogen goes down. These changes in the levels of estrogen can cause menstrual migraines and headaches. When progesterone and estrogen levels drop to it's bottom premenstrual migraines can occur.

It's noticed that migraines usually disappear when woman gets pregnant. Some researches show that more than half of women who had headaches related to menstrual cycle say that their migraines disappeared after they got pregnant. It also happens that migraines are still present in the beginning of pregnancy and they disappear after two-three months of pregnancy.

So what can be the reason of menstrual migraines? One of the reasons can be hormone replacement therapy. It's used during menopause. The other reason is birth control pills, that contain a lot of estrogen. Hormonal migraines can be decreased if birth control pills have lower doses of estrogen and usually disappear if these pills contain progesterone.

    Menstrual migraines treatment.

Menstrual migraines can be stopped by NSAIDs (Nonsteroidal anti-inflammatory medications).

Here is the list of commonly used NSAIDs: Relafen, Advil and Motrin, Naprosyn, Nalfon, Orudis.

You have to start to take NSAID medications three days before the beginning of menstrual cycle and keep taking until the menstrual period ends. If you are also taking birth control pills it's recommended to start taking NSAIDs on the l9th day of menstrual period and stop on the second day of the following cycle.

    Migraines Treatment.

When you need to relieve migraine pain and avoid new migraine attacks you need appropriate treatment for migraine headaches.

There are two ways to treat migraine - abortive and preventive.

    Abortive.

This type of migraine treatment should be taken to stop migraine attack once it's started or to prevent it. The medicine stops migraine during its prodrome stage or when it has begun, you may take it as needed. Some of them can be taken as a wafer and it will melt on the tongue, others can be self-injected into the thigh. These types of migraine treatment work quickly and helpful for people who vomit when migraine starts.

Abortive drugs include the triptans, which specifically target serotonin. They have similar chemical structure and work same way. The triptans do not relieve pain from arthritis, back problems or  menstruation. They are used to treat headache pain.

Here is the list of abortive drugs:

    Sumatriptan (Imitrex)
    Rizatriptan (Maxalt)
    Eletriptan (Relpax)
    Zolmitriptan (Zomig)
    Naratriptan (Amerge, Naramig)
    Frovatriptan (Frova)
    Almotrip

The following drugs are also affect serotonin, but they affect other brain chemicals. Also, one of these drugs works when a triptan does not.

    Ergotamine tartrate (Cafergot)
Dihydroergotamine (D.H.E. 45 Injection, Migranal Nasal Spray)
    Acetaminophen-isometheptene-dichloralphenazone (Midrin)

The following medicine usually used for nausea, but also have abortive or preventive effect on headaches:

    Promethazine (Phenergan)
    Prochlorperazine (Compazine)

The next drugs are referred to weak narcotic class. Since they can relieve any pain they are used not only for migraine treatment. Because they can form habit, they are less desirable than the specific headache drugs. These medications should be used primarily as additional treatment for the cases when a specific drug does not work.

Acetaminophen and codeine (Tylenol With Codeine)
    Butalbital compound (Fioricet, Fiorinal)

    Preventive

This type of migraine treatment is used when you have more than one migraine per week. They help to decrease severity and frequency of the migraine attacks. Medication to prevent a migraine can be taken daily. Preventive treatment medications include the following:

Antiseizure medications - Gabapentin (Neurontin), valproic acid (Depakote), topiramate (Topamax)

Medications used to treat high blood pressure - Beta-blockers (propranolol [Inderal]), calcium channel blockers (verapamil [Covera])

Antidepressants - Amitriptyline (Elavil), nortriptyline (Pamelor)

Some antihistamines and anti-allergy drugs, including diphenhydramine (Benadryl) and cyproheptadine (Periactin)

    Types of headaches.

Tension headaches - also known as chronic daily headaches or chronic non-progressive headaches. This type of headache is the most common. It may come and go over a prolonged period of time. These muscle contraction headaches cause mild to moderate pain.

Migraines - the exact causes of migraines are still unknown. Migraines are related to blood vessel contractions and other changes in the brain. Inherited abnormalities in certain brain areas also can be the reason. The pain is moderate to severe and can last from several hours to several days and occur 1 - 4 times per month. Migraines are associated with symptoms such as sensitivity to light, noise, or odors; vomiting or nausea; stomach upset or abdominal pain; loss of appetite.
